Remote Functional Intake w/ Maysa
Service Description
An hour long session with Dr. Maysa Alavi. You will discuss your history with Dr. Alavi who will develop a custom plan for your care. Your plan will have recommendations addressing: Nutrition, Lifestyle, Exercise/Movement, Sleep, Stress Management, Supplements and Laboratory follow-up. The plan will be emailed to you for your records and implementation, Any supplement prescriptions and lab recommendations will be emailed to you for easy ordering.
Cancellation Policy
CANCELLATIONS/REBOOKING To avoid a "No-Show" fee clients must cancel/reschedule at least 24 hours in advance of the visit start time by texting 929-376-2972 to notify us of the cancellation. The “No Show” fee is equal to the entire visit cost, except for Functional Lab Follow-ups only where it is $150. For clients who cancel at least 24 hours in advance we offer one complimentary rebooking per appointment. Additional re-bookings performed by us for incur a $35 fee per occurrence. You can cancel and rebook yourself in the Wix Spaces App or by logging into our website. REFUNDS Appointments cancelled with the proper notice are eligible for refund to the original form of payment, less a 5% credit card and processing fee. Alternatively, the refund may be held in full as a credit to be used at a later time, within one year. CHANGE OF CARD A request to change your credit card used after payment has already been placed incurs a 5% credit card and processing fee. PRESCRIPTION MEDICATIONS Any prescriptions ordered through our office are non-refundable.
