Service Description
A thorough evaluation to determine your eligibility for the group ketamine event. Visit includes a comprehensive intake history and physical, education, and psychedelic preparation. This visit is a prerequisite to schedule ketamine followup treatments. All forms and surveys must be completed before your visit otherwise you will be asked to reschedule. Prerequisite forms and surveys: 1) Ketamine Client Signature Forms (link sent via email) 2) Default Intake Form — this is the demographic and medical history form (link sent via email) 3) Mental Health Surveys found in the Osmind app — Please download the app to complete the surveys.
Cancellation Policy
Group Commitment By booking, participants agree to: • Attend the group session from start to finish • Participate in the included integration session(s) unless otherwise discussed in advance • Engage respectfully with the group process Repeated or uncommunicated absence, early departure, or disengagement from integration may affect eligibility for future group participation. Arrival Time Group sessions begin promptly. Participants arriving more than 15 minutes late may not be permitted to participate. Late arrival does not qualify for refund, credit, or rescheduling. Medical Screening & Eligibility Participation requires completion of all required intake forms, Osmind questionnaires, and medical screening steps to determine eligibility for the group. Drug Screening Urine drug screening is required on the day of the event. Positive urine drug screening for illicit substances or for controlled substances not prescribed to the participant may result in treatment being withheld without refund. Cannabis (Louisiana): Cannabis use must be disclosed in advance. Only prescribed medical cannabis may be permitted at clinician discretion. Non-prescribed, undisclosed, or clinically concerning cannabis use may result in treatment being withheld without refund. Pregnancy Screening Ketamine is not administered during pregnancy. Participants must confirm they are not pregnant and complete urine pregnancy testing. If treatment is withheld due to a positive test, fees are not refundable, though a credit may be issued for future services at clinician discretion. Medication & Preparation Some medications or substances may need to be held prior to treatment. Participants agree to review and follow preparation instructions and to contact the clinic in advance if holding a medication is unsafe or unclear. Transportation & Safety Participants must arrange a safe ride home. Driving or operating heavy machinery after treatment is not permitted until the following morning.. Confidentiality All personal information, experiences, and sharing during group sessions and integration spaces must remain confidential within the group. Booking, Fees & Refunds Fees are non-refundable for missed sessions, late cancellations, late arrival or ineligibility determined on the day of the event, including withholding of treatment due to screening results or failure to meet preparation requirements. By booking, participants acknowledge and agree to the policies above.
